NeighborHealth’s Pulmonary Department provides specialized care for patients with all types of lung disease. The conditions we treat include asthma, pulmonary emboli, emphysema and chronic bronchitis, lung disease, pulmonary vascular disease, lung cancer, and sleep apnea and other sleep disorders.

Asthma and COPD Home Visiting Program

You can control your asthma/COPD and we can help!

NeighborHealth wants to help you control your asthma or COPD. Asthma is a chronic, lifelong condition that inflames and narrows the airways in the lungs, making it difficult to breathe. Inhaling certain substances or irritants called “triggers” may cause frequent episodes of coughing, wheezing, chest tightness, and shortness of breath. This may also cause a flare-up or “asthma attack.”

COPD is also a disease that makes breathing difficult. COPD includes chronic bronchitis, emphysema, or a combination of both conditions. Asthma is not COPD. Our goal is not just relieving asthma/COPD symptoms but preventing them before they start!

NeighborHealth’s Asthma/COPD Home Visiting Program is a no-cost program for patients with uncontrolled asthma or COPD. Through our program, trained community health workers help patients learn to address asthma or COPD triggers at home, reduce asthma or COPD symptoms, and correctly use asthma or COPD medications.

Who Is Eligible?

To be eligible for the Asthma/COPD Home Visiting Program:

  • You must be a Pulmonary Department patient
  • You experienced at least one of the following in the last year:
    • Hospitalization for asthma or COPD
    • Frequent PCP and ED visits
    • Frequent Prednisone use
    • Frequent Albuterol use
  • You need better understanding of your disease process and medications

Our Services

  • Four home visits over a six-month period.
  • Asthma/COPD education (such as symptoms, triggers, prevention, and medication).
  • Supplies to help you keep your home “asthma/COPD safe” including:
    • Cleaning supplies
    • Vacuum or mop
    • Food-storage supplies
    • Anti-pest supplies
    • Allergen-proof covers for your mattress and pillow
  • Access to a knowledgeable, caring community health worker who will work closely with you, your pulmonologist, and your community to ensure that your pulmonary needs are addressed.

We help you:

  • Learn about asthma/COPD
  • Reduce asthma/COPD symptoms
  • Reduce asthma/COPD triggers at home
  • Learn how to correctly use asthma/COPD medications
  • Learn the benefits of an asthma action plan
  • Reduce unwanted doctor visits and hospitalizations
  • Miss fewer days of school or work
