Notice to Patients
As part of our commitment to the privacy of your health information, we would like to inform you that, as the result of a mailing error, your name and your relationship to us as our patient may have been inappropriately disclosed.
On March 9, 2024, we learned that errors in the mailing list for our announcement of our name change from East Boston Neighborhood Health Center to NeighborHealth resulted in some patients’ full names being combined with a different patient’s address. This issue affected 1% of letters, or 526 patients.
No information beyond names and the fact that the person was or is a patient of NeighborHealth was disclosed. We have no reason to believe that this information has been used to harm any individual.
We sincerely regret that this incident occurred. We have taken this opportunity to review and strengthen our procedures to ensure this never happens again.

NeighborHealth maintains medical records electronically for all patients who receive treatment or services at our facilities. Our electronic medical record system is a secure and efficient way to have your health information, including appointment notes and test results, readily available to your provider so that they can make timely decisions about your treatment.
Frequently asked questions about the release of medical information:
Requests for a copy of your medical record should be made to your primary care provider (PCP) or by speaking with someone at the greeter’s desk. Since under federal law there are specific requirements regarding the information that must be included to authorize the release of medical information, you will be asked to complete and sign a form before we process your request. You will also need to provide a photo ID so that we can verify your identity.
Usually, it takes no more than 10 business days to obtain a complete copy of a medical record. If you need a copy sooner, please let us know so that every effort can be made to meet your needs.
NeighborHealth gives you a copy of your medical record. We are required by Commonwealth of Massachusetts law to keep your original medical record on file for 20 years after the last time you were treated here.
We are not allowed to give your record to a family member, friend, or anyone other than you without your specific written permission. If you have already completed an authorization form for release of information but want someone else to pick up your record, you will still need to provide written authorization for us to release your record to that person.
Your lawyer may request the copy of your medical record directly if they want the record to be certified as accurate and complete. We do not certify medical records that are picked up by the patient or others. We need a written authorization from you or your legally recognized representative to send your medical record directly to your lawyer.
Bring in any forms from third parties you may need us to look over and sign. Or, if you need a specific provider to complete a form, please request it to your PCP. This process usually takes one to two weeks. A staff member will call you to let you know when the form is ready for pick up.
