CATCH (Children’s Access to Coordinated Healthcare)
Caring for children with varying abilities requires a high level of communication and coordination among care providers, medical specialists, social services, schools, local agencies, care centers, and families.
Coordinating our approach for complete care
- Using a team approach to overall health and development, CATCH manages each child’s care, keeping families informed and involved every step of the way. This coordinated effort helps us to keep these children as healthy as possible and support their families and caregivers.
Providing a personalized health plan
- Each CATCH participant has a unique health plan that’s created by families and care providers. The plan establishes a “medical home” as the umbrella for coordinated care, overseen by physicians and specialized nurse practitioners—which means that most participants receive care right at their own house, and only visit the health center when necessary.
Supporting families through the community
- Supporting family members is an important part of our program. We provide a community resource coordinator who organizes special events for families, acts as a parent liaison, and performs home visits as needed for our Spanish-speaking families. In all ways, we strive to encourage each child and their family to be active members of our community.
Eligibility is open to children living in the Greater Boston area who require adaptive equipment and/or advanced therapies.
